Your child's health has much to do with their success in high school. For that reason, when your child enters the ninth (9th) grade in Jimtown High School, the school requires that the student have the following forms completed and in the school office by the first day of school.

A. Physical examination by a licensed physician including:
1. Tuberculin skin test, report date and result if done.
2. Urinalysis - to be done at the doctor's office.

B. Immunizations: VACCINE CHECK LIST FOR HIGH SCHOOL STUDENTS
1. 3 doses of diphtheria-tetanus-pertussis (DTP), diphtheria-tetanus-acellular pertussis (DtaP),pediatric diphtheria-tetanus vaccine (DT) or Tetanus-diphtheria (Td)
2. 3 doses of Polio vaccine;3. 2 doses of MMR (measles, mumps, rubella) with the first dose given on or after the first birthday.
4. 3 doses of Hepatitis B vaccine.

C. Dental examination
Healthy teeth and gums are an important part of life. We encourage you, as parents, to make and follow through with regular dental check-ups for your teen-aged child.

D. If your child would like to participate in athletics any time during their ninth grade year,
an I.H.S.A.A. physical form must also be on file. Please submit both forms to the doctor on the day of your appointment, and bring both of them back to school by the first day of school. Physicals given for athletic purposes alone will not satisfy the regular school physical requirements.

E. If your child must take medication at school. Please follow the Policy For Administration of Medication at School.
