High School 9th Grade Requirements IHSAA Pre- Participation form Sports physical Consent for drug testing form • School physical required for 9th grade. • IHSAA sports physicals are required for all IHSAA recognized sports and due on or before the first day of practice. Must be done after May 1 for the up coming school year.
Junior High Sports physical form Consent for drug testing form • A sports physical is required for all competitive sports and due on or before the first day of practice.
Intermediate School Sports physical form • School physical required for 6th grade. • A sports physical is required for all competitive sports and due on or before the first day of practice.
Elementary Schools • A school physical is required for kindergarten.
