VACCINE CHECK LIST FOR HIGH SCHOOL STUDENTS

Dear Parent or Guardian of high school students:

In an attempt to help prepare your child for graduation and the vaccines he or she should have by then, I am sending home a list of vaccines that are required and/or recommended.

Please check with your child's healthcare provider, the Health Department (523­2127), or the school (295-2343) if you have questions regarding a vaccine or if

you need a copy of the vaccine record. Compare this list to your child's record.

Hepatitis B

Need to have a series of doses if not already given.

MMR
(Measles, Mumps, Rubella)

Need at least 2 doses

 

Tetanus, diphtheria (Td)
Or Tdap

Need a booster dose after age 11 (if it has been 5
years or more since last dose). After that you will need a Td booster dose every 5 -10 years.

Polio Need at least 3 doses.
Varicella

If you have not been vaccinated and have not had Chickenpox, you should get vaccinated against this disease. Teens 13 yrs and older need 2 doses.

Meningococcal
(Menactra)

Children 11 years and older can get this shot. College - bound students should get vaccinated against meningococcal disease.

I-lepatitis A If you plan on traveling outside of the U.S. you will want to contact your doctor or the health department about getting this 2-dose series of shots.
