A record of immunizations, report of physical
examination, and a legal birth document are required of
all students for initial entry and must be provided by the
parent or legal guardian within 20 days of entry.

Failure to provide a record of immunizations will be
cause for exclusion from school.

Failure to provide a legal birth document will result
in notification to the Indiana Clearinghouse for Information
on Missing Children
All students must have documentation of
immunizations against pertussis, diphtheria, tetanus,
polio, mumps, rubella, and measles. Additionally,
children in kindergarten and grades one, two, three, and
four must be immunized against Hepatitis B. Also, children
in kindergarten and grade one need to be immunized against
chicken pox or have written statements indicating when they
have had the disease. Only when a doctor certifies that
a required immunization may be detrimental to the child’s
health, or when the parent indicates in writing a religious
objection to such immunization, will the requirement be
waived.
If a parent is unable to secure immunizations, local service
providers are available to provide the immunizations. For
this service, the parent needs to contact the school nurse.
